About Us.
Case Management Professionals, Inc. opened its doors on Oahu in April 1999 with a goal of providing superior care and promoting quality of life to the elderly, aged and disabled population who are at nursing level of care but choose to reside in a residential care setting. Since then, we have established ourselves as a pioneer in the State of Hawaii as a Licensed Home and Community-Based Case Management Agency. Today we are inspired and committed and look forward for new and innovative ways to maintain the highest levels of care through consultation, placement, education and training, ancillary and contract services for our clients and caregivers.
Our staff is comprised of compassionate and highly qualified professionals with extensive clinical and social services experience. Each carefully selected team member has a strong determination to meet an individual's long-term care needs. Our case management team consists of a registered nurse and a master's level licensed social worker backed by an efficient set of administrative staff.
